Completing and submitting this form:

1. I hereby agree to the collection, storage and processing my personal data for the purpose of the Let's Dance Festival. I provide the agreement for the period essentially necessary to participate in the Festival activities.
2. I agree to using photos and video recordings of my person’s performance taken during the Let's Dance Festival activities and placing them on social network, DVDs, CDs that can be sold later on. I agree to using the photos and video recordings of my person’s performance for the purpose of marketing the Let's Dance Festival contest.
3. I agree with the terms necessary for participating in the programme of the Festival / activities organised within the scope of the Let's Dance Festival.
4. I agree to making public my name and/or the name of the group (I am a member of) on the list of participants, on the websites of the Festival, on the programme of the Festival, in the theatre projection. I also agree to making public the outcome of the contest on the websites of the Festival and social network.
